This report presents four cases of elderly ischemic cerebral infarction that achieved a beneficial effect from cilostazol that was administered for their aspiration pneumonia complaints. The diagnosis of the four cases varied depending on the basal ganglia dysfunction of each patient. Improvement of brain circulation might be involved in the beneficial effect of cilostazol. The four cases suggested that low-dose cilostazol is an effective medication for ischemic chronic stage cerebral infarction with aspiration pneumonia.
