A-68-year-old man had a tumor on the thoracic wall for two years. It had a diameter of 10cm, and was elastically hard. Histopathological findings showed fibrous tissue made of compact and mature fibroblasts and collagen fibers. Fbbroblasts are small, slender nuclei with rare mitotic figures. We diagnosed that tumor as a desmoid tumor. Tumor resection was performed; a large defect (20×20cm) in the trunk had to be reconstructed. We showed a new strategy of repairing soft-tissue defects in the thoracic wall. We utilized two flaps, which are both pedicled latissimus dorsi muscle perforator flaps. One is perforated from the circumflex scapular artery, which we call a scapular flap, and the other is perforated from the intercostal artery. We propose these flaps latissimus dorsi perforator flaps and demonstrated some points of technique for the repairing of soft-tissue defects in the thoracic wall and rising musculocutaneous flaps.
