We show a new surgical procedure of anterior maxillary segmental distraction osteogenesis for correction of the midfacial retrusion in cleft lip and palate patients. The osteotomy is performed in the anterior portion of the maxilla and the separated anterior segment is elongated forward by the intraoral distractor (DynaForm System: Stryker Leibinger GmbH & Co.KG.), which is positioned at the surface of the palate and secured trans-mucosally by screws with palatal bone. In all cases, velopharyngeal function did not show any changes after this operation. This procedure may be effective for the correction of maxillary retrusion, especially for cases that could suffer deterioration of speech following total maxillary distraction osteogenesis.
